Mobile Device Email Access Form

Users can access WSLH email using our WebMail (https://webmail.slh.wisc.edu) portal from any browser without filling out this form.  Users requesting direct access to WSLH email from mobile devices must agree to minimum security requirements and user responsibilities as outlined in the DocuSign form listed below.  Additional information can be found in LABWIDE GENOP 210, "Computer Use & Security Policy" found in OnBase.

Detailed instructions can be found in the attached document to your lower right.  Below is a general summary of the steps involved.

  1. Login to DocuSign
  2. Find and use the WSLH Mobile Device Access Form template in DocuSign
    1. Templates > Show More > Shared Folders > WSLH Templates > WSLH Mobile Device Access Form > Use

  1. In the template, enter the name of the person along with their WSLH email address, then click SEND.

  1. DONE!  The signed document will automatically be sent to the OIS Infrastructure Team so there is nothing further you need to do.
