New Account Request - New Employee

  1. Once employee has accepted a job offer,
  2. Submit a New/Modified Employee Notification
  3. Five day lead time is required after attaching completed and signed form O:\Forms\OIS\WSLH System Access Request.pdf
  4. HIPAA External Training for new to UW employees that don't yet have access to the current year Canvas HIPAA course. Required before account access can be provided.
  5. New employee will need to get a UW ID and activate their UW NetID using number from their photo ID
