The "WSLH Remote Access Authorization Form" is required before an employee can be granted remote access to WSLH systems and data (VPN/RDC) as outlined in LABWIDE GENOP 210, "Computer Use and Security Policy." In short, it states that remote access to WSLH systems and data must be authorized separately from general computer systems access and explains the responsibilities of the end user to adhere to policies and minimum security requirements.
- Users - by signing this document you agree to be responsible for adhering to all UW-Madison and WSLH policies and for ensuring the workstation being used for this remote connection meets minimum security and configuration standards (e.g. password management, virus protection, operating system and applications are patched and up to date).
- Supervisors - by signing this document you are acknowledging this user has a defined business need to remotely access WSLH systems and data and authorize this access.

- Login to DocuSign
- Find and use the WSLH Remote Access Authorization Form template in DocuSign
- Templates > Show More > Shared Folders > WSLH Templates > WSLH Remote Access Authorization Form > Use
- In the template, enter the full name and WSLH email address for the employee who needs remote access. Then enter the full name and WSLH email address of this person's supervisor and click SEND.
- DONE! The signed document will automatically be sent to the OIS Infrastructure Team and they will take it from there.
