Computer Use Agreement Form

The "WSLH Computer Use Agreement" is the attestation form associated with LABWIDE GENOP 210, "Computer Use and Security Policy" which applies to all members of WSLH’s workforce including all employees, authorized contractors/vendors, volunteers and students. Its purpose is to help users understand their personal obligations regarding confidential information, security, and the appropriate and acceptable use of WSLH computer systems and data.  This form and up-to-date HIPAA training are required prior to issuance of computer network or application credentials (user ID and password).  See OnBase for the latest copy of LABWIDE GENOP 210, "Computer Use and Security Policy."

Detailed instructions can be found in the attached document to your lower right.  Below is a general summary of the steps involved.

  1. Login to DocuSign
  2. Find and use the WSLH Computer Use Agreement Form template in DocuSign
    1. Templates > Show More > Shared Folders > WSLH Templates > WSLH Computer Use Agreement Form > Use

  1. In the template, enter the name of the person along with an existing email address so they can complete this form, and then click SEND.  Remember - this form is required before we can grant access to WSLH computer systems so you should use a non-WSLH email address.

  1. DONE!  The signed document will automatically be sent to the OIS Service Desk so there is nothing further you need to do.
